The clinical Pharmacist of Patient & Practice Solutions will be responsible for leading the transformation of practices “upstream” by implementing a hybrid model of care blending clinical intervention with operational consulting. Supporting specific regional market field teams, practices, and patients, the pharmacist drives performance for medication adherence and other pharmacy-related measures.
By engaging directly with patients, the pharmacist serves as a seamless extension of the practice care team, fostering trust and continuity. This direct patient interaction provides critical feedback, allowing the pharmacist to translate real-world patient barriers into actionable systemic workflow improvements for the practice.
The pharmacist will leverage this unique, holistic perspective and subject matter expertise to diagnose drivers of success and refine strategies for improvement. The ideal candidate is highly adaptable and thrives in dynamic environments, with a proven ability to define processes and structure work within evolving priorities. As the pharmacist will be involved in pilots and "test and learn" workflows, an ability to be nimble is key.
Candidates should be comfortable working remotely/work from home anywhere within the US.

Primary Duties:
Practice and Market Coaching
Engage with high-priority practices to drive quality improvement and service adoption. Identify practice barriers, suggesting sustainable and scalable workflow changes, and educating staff. Leverage current data to understand individual practice barriers and stakeholder management with relationship building with practices and internal market and field members.
Patient Engagement
Conduct direct patient engagement and chart review and address patient-specific barriers to medication adherence and disease state management. Prevent and resolve medication-related problems and make appropriate evidence-based, patient-centered medication and therapy recommendations back to practice care teams. Use insights gained from patient outreach to identify and translate systemic gaps in practice workflows.
Strategy & Optimization
Leverage clinical expertise and data to support business needs to refine and create standardized resources and tooling to improve medication outcomes for practices, clinicians, and patients. Identify drivers of success and refine strategies for improvement.Participate in pilot programs to "test and learn" workflowsOther duties as assigned
Minimum Qualifications:
PharmD from an ACPE-approved college of PharmacyActive, unrestricted license to practice pharmacy in, at least, two states of practice within assigned region; Willingness to obtain additional licenses if needed3 yrs experience in value-based care, community or clinical pharmacy practice, or completion of a PGY1 Pharmacy ResidencyIn depth knowledge of medication adherence, pharmacy quality measures, and medication spendProven ability to build relationships with providers and educate patients Strong critical thinking & analytical skillsProficient in Excel or Google Sheets; Ability to learn new technology Highly adaptable with the ability to structure work within evolving prioritiesAbility to organize and articulate pharmacy performance to market and senior leadersValid pharmacist license in at least 2 of the following markets and a willingness to acquire more states as needed: VA, MS, AL, LA, AR, TN, TX, GA, SC, FL
