We are seeking an enthusiastic and motivated Occupational Therapist to join a dynamic Community Reablement Team based in Dorset

This rewarding role focuses on supporting adults following hospital discharge, enabling them to regain independence and safely remain within their own homes. Working as part of a multidisciplinary team, you will assess, plan and deliver person-centred interventions to promote recovery, prevent readmission, and improve quality of life.

The role is predominantly community-based and will involve independent lone working across Bournemouth, Christchurch and Poole.

Key Responsibilities

  • Assess the functional, physical and cognitive needs of adults following hospital discharge.
  • Develop and implement individualised reablement and rehabilitation plans.
  • Recommend equipment, adaptations and support services to maximise independence.
  • Work collaboratively with healthcare professionals, social care teams, patients and carers.
  • Manage a varied community caseload and prioritise workload effectively.
  • Complete comprehensive assessments, documentation and risk assessments.
  • Promote safe discharge pathways and prevent unnecessary hospital readmissions.
  • Participate in multidisciplinary meetings and contribute to service development.
  • Adhere to HCPC standards, professional guidelines and local policies.

Essential Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC Registration.
  • Experience working within community, rehabilitation, reablement or acute healthcare settings.
  • Strong assessment, communication and clinical reasoning skills.
  • Ability to work independently and confidently in a lone-working environment.
  • Full UK driving licence and access to a vehicle for work purposes.
  • Excellent organisational and time management skills.

Desirable Experience

  • Previous experience within Community Reablement services.
  • Experience supporting adults transitioning from hospital to home.
  • Knowledge of equipment provision, moving and handling, and home adaptations.
  • Experience working within multidisciplinary teams.
