About the Role: The Community & Patient Liaison is a unique hybrid role that combines traditional clinic support with referral coordination, provider relationship management, and community outreach. The goal is to create an exceptional patient and provider experience while helping grow the Cancer Center through strong local engagement.

 

Responsibilities:

Clinic Operations

  • Prepare clinic and support oncologist during clinic days
  • Room patients, obtain vitals, and assist with patient flow
  • Coordinate labs, imaging, treatment orders, and follow-up appointments before the patient leaves
  • Schedule infusions, office visits, and outside referrals
  • Track outstanding tests and ensure patients stay on their care plan
  • Serve as the primary local clinical support resource

Referral Coordination

  • Obtain medical records and required documentation as necessary
  • Establish and maintain external referral relationships
  • Follow up on pending referrals and referral barriers
  • Communicate appointment status back to referring offices
  • Maintain referral activity within the CRM/EMR
  • Ensure a seamless referral experience for providers and patients

Community Provider Outreach

  • Visit referring provider offices on a regular cadence
  • Build relationships with PCPs, specialists, surgeons, hospitalists, ED providers, office managers, and referral coordinators
  • Educate practices on referral processes and Cancer Center services
  • Deliver updated marketing materials
  • Provide referral appreciation gifts (cards, chocolates, etc.)
  • Identify opportunities and barriers to referral growth

Marketing & Community Engagement

  • Support provider lunches and educational events
  • Participate in community outreach activities
  • Coordinate with central marketing on local campaigns and collateral
  • Support local online presence and Cancer Center awareness initiatives

Performance & Reporting

  • Track referral volume and conversion
  • Monitor time from referral to first appointment
  • Document community outreach activity
  • Identify referral trends and improvement opportunities
  • Provide regular updates on provider engagement and growth initiatives

Success Measures

  • High patient satisfaction and efficient clinic operations
  • Strong relationships with community providers
  • Increased referral volume and reduced referral leakage
  • Timely referral processing and scheduling
  • Consistent provider outreach and engagement

 

Qualifications:

  • Certified Medical Assistant, Certified Nursing Assistant or Licensed Practical Nurse required
  • Clinical experience in a primary care, specialty care or ambulatory healthcare setting
  • Proficiency in using electronic health record systems for documentation, scheduling and care coordination
  • Experience providing patient education and presenting to community groups
  • Ability to support outreach, education and engagement initiatives for new clinical programs or service lines
  • Demonstrated commitment to community engagement and access to care
  • Excellent interpersonal and communication skills to build rapport with physicians, patients and community partners
  • Ability to work collaboratively with clinical and administrative teams
  • Compassion for delivering patient-centered care and building community health initiatives
