Our care model combines in-clinic and virtual care to bring support to patients where and when they need it.Backed by $50M in fundingāincluding a $25M round led by CVS Health Ventures with support from a16z Bio + Health, Flare Capital, MassMutual, and Intermountain Venturesāweāre scaling fast and expanding access to evidence-based obesity care nationwide.As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APCs, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.Schedule: Monday- Friday with flexible scheduling, 32 patient contact hours minimum (+ 8hrs of administrative time).āÆ Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-CallEMR: AthenaOn-Call: Phone call only, in a rotation for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8.1.24). 24/7/365 physician back up.Location: Hybrid in Plano, TXResponsibilities:
- Practice exceptional medicine within knownwell
- Sees adults for metabolic health, and primary care services as isappropriate in a virtual setting. This may include sharing a panel with MD/DO + other APCs of the local in-clinic practices.
- May see pediatric patients ages 12+ for metabolic health only, based on training, licensure, and competency.
- Remote care using audio and visual technology to complete patient examinations, evaluation, assessments, diagnoses and treatment plans.
- Embodies the knownwell mission and values of patient inclusivity in how you deliver care.
- Coordinates with the broader care team to ensure the patient has the support they need.
- Proficiency with Microsoft Office applications recommended, but not required.
- Documents as necessary to ensure accurate billing, including review of remote patient monitoring data.
- Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response.
- Ensure patients feel known well
- Ensure your patient panel is open to any individual, regardless of their current health status and comorbidities.
- Listen to the aspirations of the patient and ensure care is tailored to the patientās unique health journey.
- Provide an environment in which patients feel comfortable and free from judgment.
- Offer exceptional encouragement as patients reach their goals.
- Coordinate across the collaborative care team
- Escalate any concerns to your dedicated site leadership team.
- Provide clinical guidance to medical assistants and nursing teams as assigned.
