Band 8a Locum Pharmacist needed in Nottingham.

20 hours per week (a Tuesday afternoon must be covered)
 
We have a 3 month position for someone within our Integrated Care Home Service for 20 hours per week, commencing as soon as possible. One day needs to be a Tuesday.
 
The pharmacist role within the service is to carry our medication reviews of new residents to care homes, who have been assessed by a specialist nurse practitioner. Attend a weekly MDT with a consultant geriatrician, specialist nurses, on a Tuesday afternoon to be involved in the discussion around the patient and ongoing medical management. Up to date pharmacological knowledge is essential around medicine management of conditions common within the older population.
