Role Scope: This is an opportunity for a mission-driven team player who is passionate about attending to the daily living and safety needs of an assigned group of participants in our PACE center and occasionally in the home.  By performing various participant care activities and related non-professional services, you will ensure our participants’ physical, social, and recreational needs are met so that they have the best experience possible when they are in our Habitat Health Center. 

Core Responsibilities & Expectations for the Role 

This is an opportunity for a mission-driven team player who is passionate about attending to the daily living and safety needs of an assigned group of participants in our PACE center and occasionally in the home.  By performing various participant care activities and related non-professional services, you will ensure our participants’ physical, social, and recreational needs are met so that they have the best experience possible when they are in our Habitat Health Center.  

Core Responsibilities & Expectations for the Role 

  • Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong.  
  • Continue to raise the bar.  Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants.

  • Meets participant’s personal hygiene and basic needs in a timely and caring manner.  
  • Assists participants with activities of daily living, such as feeding, ambulation and other activities.  
  • Responds to and fulfills participant request or obtains appropriate assistance.  
  • Performs and assists with assigned non-sterile procedures and other assigned tasks in support of participant care.  
  • Prepare and plate food for meal service, following instructions on participants’ dietary cards for specific diets, preferences, and special needs.  
  • Collects data and records activity and participant care information in the EMR.  Ensures that information is accurate, timely and recorded according to established guidelines.  
  • Cleans and straightens immediate participant surroundings ensuring that the participant is comfortable, necessary supplies are available and within reach, and physical privacy is maintained.  
  • Assists in maintaining an orderly, clean participant care unit by straightening supply and linen storage. 
  • On occasion, provides homemaking services in the home including preparing and serving meals and snacks according to specified nutritional needs and dietary restrictions as outlined on the plan of care.  Performs light housekeeping tasks as outlined on plan of care including vacuuming, mopping, dusting, taking out trash, washing dishes and doing laundry.  
  • Accompany participants to outside appointments and conduct appointment reminder calls in advance of scheduled visits. 
  • Observes participants for emotional, behavioral, and physical changes in condition including, but not limited to any changes in mood, skin integrity, bowel and bladder status, food/fluid intake, speech, ambulation, and transfer ability.  
  • Monitors participant-wandering alert devices, responds to elopement risk alerts, and follows facility safety escalation procedures. 
  • Supports participant activity engagement, including vendor-led activities, under Activity Coordinator oversight. 
  • Conducts daily safety checks (e.g., call light/alert systems, water temperatures) and responds promptly to any issues identified. 
  • Performs related duties as assigned.  

Required Qualifications:  

  • Completion of accredited vocational nursing assistant course, hospital, or comparable participant care experience.  “Comparable experience” should include skills such as:  1) TPR; 2) BP; 3) bed bath; and 4) ADL.  
  • High School diploma/GED  
  • Knowledge of PACE program.  
  • Food Handler's Certification required; candidates without current certification at time of hire must obtain it within 30 days of start date. 
  • Ability to work without close supervision or professional guidance and to exercise independent judgment.  
  • Knowledge of physical, mental, and social needs of frail older adults.  
  • Able to manage changing priorities per participant needs.  
  • Effective interpersonal skills to work with participants, caregivers, and staff.  
  • Ability to work sensitively and effectively with individuals of diverse ethnic and cultural backgrounds.  
  • Ability to be self-directed and able to work independently.  
  • Requires physical strength to perform essential functions of the job (i.e. assist participants with mobility challenges).  
  • A minimum of one year experience working with the frail or elderly.  
  • Proof of medical clearance for communicable diseases, including a TB test.  
  • Proof of all immunizations are up to date.  
  • Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.  
  • Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.

Compensation:

The anticipated base salary range for this position is $22.00- 26.00. This position is also eligible for an annual bonus and may be eligible for additional incentive compensation, as applicable. 

The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs. 

In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays.
