About the Role:

As the Provider Relations Specialist, you will advance Cityblock’s brand, reputation and partnerships in the communities where our members live to ensure members receive timely, high quality care and are well engaged with Cityblock.  

Reporting to the Network Strategy Lead while supporting Cityblock’s market leadership team in the Ohio Market,  you will play, develop and execute a community engagement strategy to 1) develop Cityblock’s brand and reputation in the community; 2) build strong relationships with providers/social services organizations and agencies, and third party vendors to support coordinated care and referral pathways for Cityblock’s members; 3) manage all Provider events in the Market 4) manage provider engagement as a means for meeting member needs, building trust in community and developing key relationships with external partners. 

You will partner with Cityblock’s care teams on the ground in your market,  to understand the needs of the members, how strategic provider partnerships may address those needs, and how our care team members can collaborate to make and manage referrals to provider and community organizations. 

You will also partner closely with other leaders on the Engagement team to develop and execute brand strategies to better engage members and their families. 

 

Your work will be centered around: 

Provider Relations and Partnerships

  • Identify provider/community needs and foster relationships with provider/community organizations that provide services to meet identified needs.
  • Identify, develop, and manage provider partner relationships; work with provider partners to compose data-driven strategic goals for collaborative initiatives.
  • Maintain frequent, ongoing communication with provider partners to ensure opportunities are properly identified.
  • Work with Network  to implement program and partnership strategies, an evaluation framework, and key performance indicators to measure progress.

Care Team Support 

  • Support care team members in troubleshooting navigation of available resources
  • Manage and maintain provider partner databases and manage communication workflow.
  • Liaise internally to ensure that member-facing teams are on-boarded and trained effectively to be able to leverage the Provider/CBO network and deploy provider/community resources appropriately.
  • Inform member outreach strategies and approaches based on information gathered from formal and informal provider/community engagement.
  • Strong awareness of public payer policy to support care teams in maintaining members’ eligibility for Medicare and Medicaid and enrollment in other entitlement programs they may be eligible for. 

Cityblock Engagement 

  • Support leadership team and care team members in identifying, nurturing and maintaining relevant provider partnerships in high density areas where members reside 
  • Coordinate seamless referral workflows with providers and community based organizations to address physical health, behavioral health & social needs
  • Develop strong metrics to monitor and evaluate Cityblock’s impact to community to include but not limited to: closed loop referrals,  members served, integrated partnerships
  • Create innovative opportunities for collaborating with high value provider partners in the OH Market

 

Requirements for the Role:

  • Bachelor’s Degree 
  • 2 years of experience in provider network engagement/relationships work or in a health plan, provider organization or other healthcare setting
  • Effective communicator to internal teams and external partners
  • Ability to develop collaborative relationships quickly 
  • Ability to travel within the OH Market
