The PACE Center Intake Coordinator is responsible for facilitating the enrollment of participants into the PACE program, ensuring compliance with program regulations, and coordinating care between various healthcare providers.
Responsibilities
- Coordinate and conduct initial intake screenings with potential PACE program participants, including determining eligibility and collecting necessary information for enrollment
- Coordinate with various healthcare providers to ensure seamless transition of care for participants
- Maintain accurate and complete participant records, including medical history, care plans, and medication lists
- Work with interdisciplinary team to develop individualized care plans for each participant
- Provide education and support to participants and their families regarding the PACE program and available resources
- Ensure compliance with program regulations and standards
- Participate in quality improvement initiatives and maintain knowledge of current healthcare regulations and trends
Qualifications
- Bachelor's degree in healthcare, social work, nursing or related field preferred
- Minimum of two years of experience in healthcare service coordination or case management required
- Knowledge of Medicare and Medicaid regulations preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of an interdisciplinary team
- Strong organizational and time-management skills
- Ability to maintain confidentiality and exercise good judgment
- Proficiency in Microsoft Office and electronic health records (EHR) systems
Location
- Los Angeles required. Ability to commute to Alhambra required.
- Relocation benefits available.
Salary & Benefits
- Salary: $26.00 – $30.00/hr
